Plant Milk Graders’ (PMG) Certificate Renewal Form

Plant Milk Graders’ Certificates are issued by the Ontario Ministry of Agriculture, Food and Agribusiness (OMAFA) to successful candidates. As such, individuals hold the certificate as opposed to the company they are employed with. ODC collects home/personal information in order to be able to contact individuals directly rather than the company. ODC will only contact individuals at home addresses to deal with education programs and PMG renewal information.
